Update from Administrator – December 17, 2020
Dear Family Members and Responsible Parties,
I hope this email finds you well.
There are a few attachments that we promised to send as soon as we received information from CVS regarding the vaccine clinic.
- Family Newsletter and Vaccine Updates
- COVID vaccine EUA fact sheet
- COVID vaccine responsible party form if you want your loved one to receive the vaccine
- Declination Form if you don’t want your loved one to receive the vaccine
We would appreciate if you can complete the responsible party form or the declination form and return it to the facility by Tues. 12/22/2020 so we can request the number of vaccines needed for the clinic.
The form can be scanned to administrator@newtownrhcc.com or mailed if you choose to do so. The address is at the bottom of this email.
We will hold our Weekly Phone Call tomorrow at 1 p.m.
